Flat $449 per month

Level 1
$4,900 per year (50% Down payment yearly then monthly)
- Anytime day or evening access (call or text) as primary or secondary physician (if you already have a primary Dr)
- Annual Physical Exam with Basic Screening blood laboratory testing : complete metabolic profile—>kidney & liver function labs electrolytes, complete blood count, thyroid, cholesterol, diabetes testing with hemoglobin A1C, STI testing, Urinalysis
- Anytime access = from 7 AM to 10 PM for routine non-urgent questions or concerns. If deemed emergent then call 911

Note: Difference Between Screening and Diagnostic tests:
Screening tests are used to detect early disease or risk factors in large groups of healthy or asymptomatic people. They do not give a definite answer and may lead to the need for a diagnostic test. Diagnostic tests are used to establish the presence or absence of disease as a basis for treatment decisions in symptomatic or screen positive individuals. They give a precise and accurate picture of the disease and its intensity.
Fitness Wellness Package
$6,000 per year

- Medical consultation for moderate to strenuous exercise clearance prior to starting an exercise program

This package can be added to any of the above memberships
Membership pricing may be monthly with 50% down payment on a yearly basis
Note: Tests do not take away the need for typical standard cancer screening such as mammograms, colonoscopies, skin checks and pap smears which should be done by the respective specialties
It is recommended that members/patients still keep health insurance for several reasons: major illnesses, hospitalizations, medications, or other tests that may be needed for acute problems aside from the tests that are a part of the membership screening exams. Other diagnostic tests may also be needed if the history or screening tests suggest the need for further testing.
Hence, any tests ordered aside from the screening tests throughout the year , the member will be responsible for and may use their insurance to get reimbursed.
